
CMS Announces 2026 Medicare Updates: A Step Forward for Connected Care

CMS has finalized its 2026 Medicare Physician Fee Schedule (PFS), bringing forward new opportunities for providers focused on connected care. The final rule outlines updates that impact how Remote Patient Monitoring (RPM) and other care management programs are delivered and reimbursed.
The Centers for Medicare & Medicaid Services (CMS) has released the 2026 Medicare Physician Fee Schedule (PFS) Final Rule, bringing several important updates that will influence how providers deliver and manage remote patient care.
As part of these updates, CMS finalized two previously proposed CPT codes for Remote Patient Monitoring (RPM), expanding clinical flexibility and supporting long-term program sustainability:
- CPT 99445 — covers 2–15 days of RPM data collection
- CPT 99470 — covers the first 10–20 minutes of RPM management
These new codes create greater opportunity for providers to tailor monitoring and outreach to each patient’s unique needs, supporting timely intervention, better engagement, and continued program growth.
In addition to RPM, the 2026 Final Rule includes updates to:
- Advanced Primary Care Management (APCM) services
- Federally Qualified Health Center (FQHC) and Rural Health Clinic (RHC) billing
- Preventive and wellness services, including AWVs
- Virtual care compliance and documentation requirements
